This isn't realistic for NIH, but could work at the NSF (which has a much clearer basic science mission):
How's about a single-grant cap of $100,000/year and a prohibition on grant concurrency? $100k / year is enough to keep a small program afloat, and this would insure that the total grant pool was spread out beyond the "rich" programs.
